A ruptured ACL, which refers to a complete tear, is a serious injury that often requires surgical intervention.

What Is the ACL?

The ACL has two essential functions:

The ACL works together with the posterior cruciate ligament (PCL), medial collateral ligament (MCL), lateral collateral ligament (LCL), menisci, cartilage and surrounding muscles to keep the knee stable.

How Do ACL Tears Happen?

ACL tears most commonly occur during non-contact sports injuries, but they can also result from trauma. Typical mechanisms include:

  • Sudden changes in direction

  • Pivoting on a planted foot
  • Landing awkwardly from a jump
  • Direct contact, such as a football tackle
  • Motor vehicle accidents

It is important to differentiate between an ACL sprain, which involves stretching of the ligament without rupture and an ACL tear, which involves partial or complete disruption of the ligament fibers. Partial tears may present differently from complete tears and can be identified on imaging. A mild injury may involve knee instability or a sensation of giving way and may not require surgery, but it still requires medical evaluation.

In traumatic injuries, other ligament damage such as MCL tears may occur alongside ACL tears. MRI is important for identifying associated injuries and guiding comprehensive knee injury management.

Certain anatomical and physiological factors can make some athletes more susceptible to ACL injuries. Contributing factors may include differences in hip anatomy, neuromuscular control, ligament laxity and muscle strength patterns.

ACL tears are classified by severity:

  • Grade 1: Mild sprain with microscopic tearing
  • Grade 2: Partial tear
  • Grade 3: Complete rupture of the ligament

Grade 3 injuries represent a complete ACL tear or rupture, involving full disruption of the ligament fibers. A complete ACL tear typically does not heal on its own, and surgical reconstruction is frequently recommended, especially for active patients or those with significant instability.

ACL Tears and Knee Stability

Physical examination maneuvers commonly used to assess ACL integrity include the Lachman test, pivot shift test and anterior drawer test. The anterior drawer test is performed with the knee at 90 degrees of flexion and assesses for increased anterior tibial translation, which may indicate an ACL tear.

Imaging studies are crucial for confirming the diagnosis and assessing the extent of injury. MRI is the preferred imaging modality because it can clearly show whether the ACL is intact or torn and identify associated injuries, such as meniscus tears or other ligament damage.

ACL Tear Treatment Options

Non-Surgical Treatment

Not every ACL tear requires surgery. Conservative treatment may be appropriate for patients who:

  • Do not participate in pivoting sports
  • Have minimal instability
  • Are older or medically higher risk for surgery

Non-surgical care may include:

  • Bracing to provide stability
  • Temporary use of crutches
  • Anti-inflammatory medication
  • Structured physical therapy
  • Neuromuscular training to improve knee control

However, a completely torn ACL will not heal on its own. Non-surgical care focuses on strengthening surrounding muscles to compensate for ligament instability.

ACL Reconstruction Surgery

For active individuals or those with significant instability, anterior cruciate ligament reconstruction is often recommended. Surgery does not repair the torn ligament by stitching it together. Instead, it reconstructs the ACL using a graft. Meniscal repair may be performed at the same time if needed.

Common graft options include:

  • Patellar tendon autograft
  • Hamstring tendon autograft
  • Quadriceps tendon autograft
  • Allograft, or donor tissue

Each graft type has advantages and considerations. Your orthopedic surgeon will help determine the most appropriate option based on your anatomy, activity level and goals.

ACL reconstruction is typically performed arthroscopically using small incisions. The surgeon removes the torn ligament, prepares bone tunnels in the femur and tibia and secures the graft in an anatomic position. Over time, the graft incorporates and functions as a new ligament.

Rehabilitation After ACL Surgery

Rehabilitation is a critical component of successful ACL reconstruction. Recovery is structured in phases.

Early Phase

  • Control swelling
  • Restore full knee extension
  • Regain basic mobility
  • Begin gentle strengthening

A physical therapist guides patients through recovery and exercise progression during this phase.

Intermediate Phase

Most patients return to sports several months after surgery, depending on healing, strength recovery and functional testing results. Returning to sports too early increases the risk of graft failure or reinjury.

Long-Term Outlook

With modern surgical techniques and dedicated rehabilitation, long-term success rates for ACL reconstruction are high. Most patients regain knee stability and return to prior activity levels.

However, ACL injuries increase the long-term risk of developing knee osteoarthritis, especially if associated with meniscus or cartilage damage. Early diagnosis, appropriate treatment and strength training help protect long-term joint health.

Injury prevention strategies, such as neuromuscular training and proper landing techniques, may help reduce the risk of future ACL injuries. Knee braces may provide support during return to sports, although their effectiveness in preventing ACL injuries remains under discussion.
