Shoulder Instability and Dislocations

Shoulder instability occurs when the structures that normally keep the shoulder joint in place become stretched, torn or weakened. As a result, the ball of the upper arm bone (humerus) may partially slip out of the socket (subluxation) or completely dislocate.

Because the shoulder has the greatest range of motion of any joint in the human body, it is also one of the most vulnerable to instability and dislocation injuries. These injuries can occur suddenly from trauma or develop gradually from repetitive motion or weakened supporting tissues.

Understanding Shoulder Anatomy and Stability

The main shoulder joint is the glenohumeral joint, a ball-and-socket joint where the head of the humerus fits into a shallow socket in the scapula called the glenoid cavity.

  • The labrum, a cartilage rim that surrounds and deepens the glenoid cavity
  • Shoulder ligaments, which connect bone to bone and stabilize the joint
  • Rotator cuff muscles and tendons, which stabilize and control movement
  • The shoulder capsule, a soft tissue envelope surrounding the joint that helps maintain the position of the humeral head

The glenoid rim is the bony margin of the socket where the labrum attaches, and injuries to this area can contribute to instability.

The rotator interval is an anatomical region between the supraspinatus and subscapularis tendons that also contributes to shoulder stability.

Because the shoulder socket is relatively shallow, these soft tissues play a critical role in keeping the humeral head centered in the joint during movement.

When these stabilizing structures become damaged or stretched, the shoulder may become unstable.

What Is Shoulder Instability?

Shoulder instability refers to a condition in which the shoulder joint repeatedly slips out of its normal position.

This may occur as:

  • Shoulder subluxation – a partial dislocation where the ball partially slips out of the socket
  • Dislocation – a complete separation where the ball comes fully out of the socket

Many patients describe shoulder instability as the shoulder “giving way,” “slipping,” or “feeling loose” during movement.

Repeated instability can lead to ongoing pain, weakness and a higher risk of additional dislocations.

Types of Shoulder Dislocations

Shoulder dislocations are typically classified based on the direction the humeral head moves when it leaves the socket.

Anterior instability is the most common type of shoulder instability and occurs when the humeral head moves forward out of the glenoid. Posterior instability is less common but may occur in certain situations such as trauma, seizures or electrical injury.

If underlying structural issues are not addressed after a dislocation, recurrent instability can develop, leading to repeated episodes of the shoulder dislocating or subluxating.

Anterior Dislocation (Most Common)

In an anterior dislocation, the humeral head moves forward out of the socket. This often occurs after a fall, collision or sudden force applied to the arm, especially when the arm is in abduction and external rotation.

Anterior dislocations account for most shoulder dislocation injuries.

Posterior Dislocation

Posterior dislocations occur when the humeral head moves backward. These are less common and may occur after seizures, electrical injury or high-energy trauma.

Inferior Dislocation

In rare cases, the humeral head moves downward beneath the socket. These injuries typically occur during severe trauma.

Causes of Shoulder Instability and Dislocations

Several factors can lead to shoulder instability.

Common causes include torn or stretched ligaments, injury to the joint capsule or damage to the labrum. These injuries compromise the stability of the shoulder and increase the risk of dislocation or subluxation.

Multidirectional instability is a condition in which the shoulder can dislocate in multiple directions, often due to generalized ligament laxity. This type of instability may occur even without a traumatic event and is often treated with conservative therapy.

Traumatic Injury

A sudden injury can force the shoulder out of its socket, damaging stabilizing structures such as the labrum, ligaments or rotator cuff.

Common causes include:

  • Falls onto an outstretched arm
  • Sports collisions or tackles
  • Motor vehicle accidents
  • Direct blows to the shoulder

Repetitive Overhead Motion

Athletes who frequently use overhead motions, such as throwing, swimming or tennis, may gradually stretch the ligaments and capsule, leading to instability.

Labral Tears

A labral tear can make the shoulder less stable and contribute to instability. Injuries such as Bankart lesions often occur after dislocations and increase the risk of recurrent instability.

Genetic or Structural Factors

Some individuals naturally have looser ligaments or shallow shoulder sockets, making them more prone to shoulder instability. Generalized ligament laxity, sometimes described as being double-jointed, can increase the risk of instability.

Rotator Cuff Injury

Damage to the rotator cuff muscles or tendons may reduce dynamic stability in the shoulder joint.

Symptoms of Shoulder Instability or Dislocation

Symptoms vary depending on the severity of the injury and whether the instability developed over time or resulted from a recent trauma.

Common symptoms include:

  • A shoulder that slips out of place or feels unstable
  • Sudden shoulder pain after injury
  • Visible deformity following a dislocation
  • Swelling or bruising
  • Weakness in the arm
  • Limited range of motion
  • A sensation of popping, catching or shifting in the joint
  • Numbness or tingling in the arm in some cases

Some patients experience repeated dislocations during normal activities, sports or even during sleep.

Diagnosing Shoulder Instability

Diagnosis begins with a comprehensive physical examination by an orthopedic specialist.

During the evaluation, your provider will:

  • Review your symptoms and medical history
  • Evaluate shoulder movement and strength
  • Perform stability tests to determine how easily the joint shifts

Imaging studies may also be used to confirm the diagnosis and evaluate the extent of injury.

Common diagnostic tools include:

  • X-rays to identify dislocations or fractures
  • Magnetic resonance imaging (MRI) to evaluate soft tissue injuries such as labral tears or ligament damage
  • CT scans when detailed bone imaging is needed

These tests help determine the underlying cause of instability and guide treatment decisions.

Non-Surgical Treatment for Shoulder Instability

Many patients improve with conservative treatment, particularly after a first-time dislocation. A structured exercise program is often recommended to improve shoulder stability and function.

Reduction of the Dislocation

If the shoulder is dislocated, a physician will gently guide the humeral head back into the socket, a process known as closed reduction.

Immobilization

A sling may be used for several weeks to allow injured tissues to heal.

Medications

Nonsteroidal anti-inflammatory drugs (NSAIDs) may help reduce pain and swelling.

Physical Therapy

Rehabilitation focuses on strengthening the rotator cuff and shoulder blade muscles to improve stability and reduce the risk of future dislocations.

Physical therapy is a critical part of recovery for many patients.

Surgical Treatment for Shoulder Instability

Surgical treatment may be recommended if:

  • Shoulder instability persists despite rehabilitation
  • Dislocations occur repeatedly
  • Significant labral or ligament damage is present
  • High levels of shoulder stability are required for sports or work

Many procedures are performed using arthroscopic techniques, which are minimally invasive and involve small incisions and specialized instruments to repair internal shoulder structures such as the labrum or rotator cuff.

In more complex cases, open surgery may be required to repair damaged tissues.

Common surgical procedures include:

Bankart Repair

A Bankart repair reattaches the torn labrum to the glenoid rim and tightens the surrounding ligaments to restore shoulder stability.

Capsular Tightening (Capsular Plication)

This procedure tightens the joint capsule to reduce excessive joint looseness.

Repair of Associated Injuries

During surgery, the surgeon may also repair damaged structures such as ligaments, tendons, cartilage or shoulder muscles.

Most arthroscopic procedures allow smaller incisions, less tissue disruption and faster recovery compared to traditional open surgery.

Recovery and Rehabilitation

Recovery from shoulder instability treatment depends on the severity of the injury and whether surgery is required.

Typical recovery steps include:

  • Wearing a sling for several weeks
  • Gradual restoration of range of motion
  • Physical therapy to strengthen the shoulder
  • Progressive return to sports and activity

Full recovery may take several months, particularly after surgical repair.
